New changes to the District’s Medicaid Program
Beginning May 1, 2008, DC Healthy Families is adding a new health plan to the
program. You may now choose from
Unison
Health Plan,
Health Right
or DC Chartered
Health Plan.
Your benefits will stay the same. However, here are a few important things you
should know:
- To stay in your current health plan – you
do nothing
- To change to another plan
- Contact DC Healthy Families by June
2, 2008 at (202) 639-4030, or visit the DC Healthy Families
Enrollment office at 1820 Jefferson Place, NW, from 8:00am to 9:00pm,
where our trained staff can help you in the language you speak, or
- Fill out a
Plan Selection Form and mail it or fax it

Important Information for AMERIGROUP Members
After June 30, 2008, AMERIGROUP will no longer be a health plan in the DC
Healthy Families (Medicaid) program. This means that you will need to
choose another health plan for your medical care by June 2, 2008. There
are three health plans you can choose from: Unison
Health Plan,
Health Right
or DC Chartered
Health Plan. If you do not choose a plan by June 2nd, a computer
will choose one for you. For information about each plan or to sign up:
- Call the Enrollment office, (202)
639-4030, from 8:00 a.m. to 9:00 p.m. Our call center staff are
available to help you with your selection, or
- Visit the DC Healthy Families Enrollment
office at 1820 Jefferson Place, NW, from 8:00 a.m. to 9:00 p.m., where our
trained staff can help you in the language you speak, or
- Complete a
